What is breast reduction?

Breast reduction surgery removes excess glandular tissue, fat and skin to reshape and reposition the breast. The procedure relieves the physical and postural symptoms of macromastia — chronic back, neck and shoulder pain, intertrigo (skin irritation under the breast), grooves from bra straps, and limitations on physical activity. It is also performed for asymmetry correction. Australia is one of the few countries where breast reduction has a clear Medicare pathway when symptoms and tissue-removal thresholds are met, making it markedly more accessible than many cosmetic procedures.

How breast reduction works

Performed under general anaesthetic over 2.5–4 hours. The most common technique is the inferior-pedicle Wise-pattern (anchor-scar) reduction, in which the nipple-areola complex is kept attached to a vertical strip of breast tissue (the pedicle) that maintains its blood and nerve supply, while excess tissue is removed from above and either side. Smaller reductions may use vertical-scar (lollipop) techniques. Very large reductions (>1500g per side) may require free-nipple grafting. Drains are used selectively. A surgical bra is worn for 4–6 weeks.

Who is breast reduction for?

Patients with documented symptoms of macromastia who have failed conservative measures (good supportive bras, weight optimisation, physiotherapy). Stable body weight is preferred — significant weight loss or gain after surgery alters the result. Smoking dramatically increases wound-healing complications and surgery is generally deferred until at least six weeks of cessation. The seven-day AHPRA cooling-off period is mandatory.

    Search by the practitioner's full name and select Profession: Medical Practitioner. The register returns active registrations only — if the name does not appear, do not proceed.

    Confirm the specialty (or its absence)

    Look at the "Specialty" field. A FRACS specialist plastic surgeon will list "Specialist Surgery — Plastic Surgery". Anyone who calls themselves a "cosmetic surgeon" without a recognised specialty is a general medical practitioner under AHPRA — that is a meaningful difference for surgical work.

Is breast reduction Medicare-rebatable?

Yes, breast reduction may attract Medicare item number 45523 when documented symptoms (back, neck and shoulder pain, intertrigo, postural changes, inability to participate in normal activities) and minimum tissue-removal criteria (at least 500g per breast) are met. A formal referral pathway and pre-authorisation with your private health fund is required. Discuss eligibility with your GP and surgeon.

Will I be able to breastfeed?

Many patients can still breastfeed after reduction, but a meaningful minority have reduced or no ability. The technique used (preserving the inferior pedicle and ducts) influences this. Patients planning future pregnancies should weigh this carefully.

Will my nipples still have sensation?

Most patients retain nipple sensation, although it is often temporarily altered for several months. A small percentage have permanently reduced sensation. Free-nipple-graft techniques (used in very large reductions) result in permanent loss of sensation and inability to breastfeed.

What scars should I expect?

The standard result is an anchor-shaped scar (around the areola, vertically down to the inframammary fold, and along the fold) or a lollipop scar (without the horizontal component) for smaller reductions. Scars mature over 12–18 months and typically settle to thin pale lines but can be hypertrophic in some patients.

What qualifications should my surgeon have?
