Deep plane facelift in Australia.

๐Ÿ’ฐ Deep plane facelift cost guide 2026 โ†’

A deep plane facelift is a surgical technique that releases the retaining ligaments below the SMAS to reposition deeper facial soft tissues vertically, producing the most durable and natural-looking facelift result currently available. It is the technique of choice for established mid-face descent and jowling. Performed under general anaesthetic in an accredited hospital and regulated under the AHPRA July 2023 cosmetic-advertising guidelines.

Editorial guide

What is deep plane facelift?

Deep plane facelift is the most technically advanced facelift technique in routine clinical use. It releases the retaining ligaments of the midface (zygomatic, masseteric and mandibular cutaneous) and elevates the entire midface and lower face as a single composite block. Because the soft tissue moves as a unit, the result looks natural in animation and avoids the 'pulled' appearance associated with skin-tension lifts. It is the technique of choice for established midface descent, deep nasolabial folds, and jowling.

How deep plane facelift works

Performed under general anaesthetic over 4โ€“6 hours. Incisions are similar to standard facelift (temple, in front of ear, behind ear, into occipital scalp). The skin flap is elevated above the SMAS, the SMAS is incised, and dissection enters the sub-SMAS deep plane. The retaining ligaments are released, allowing the entire midface to mobilise as a composite flap. The flap is repositioned vertically and secured. Skin is closed under no tension. A neck lift is almost always combined.

Who is deep plane facelift for?

Healthy non-smokers in their 50sโ€“70s with established midface descent, jowling and neck laxity. Younger patients with early changes are typically better served by mini-facelift or non-surgical maintenance. Smokers must abstain at least six weeks pre-surgery. Patients with significant cardiovascular comorbidities or untreated body dysmorphic disorder are not candidates. The seven-day AHPRA cooling-off period is mandatory.

What drives the cost

Typical Australian cost ranges from $25,000 for deep plane facelift alone through to $45,000+ for combined deep plane + neck lift + brow lift + blepharoplasty. Quotes should be itemised: surgeon, anaesthetist, hospital, garments, follow-up.

Recovery and downtime

Hospital stay 1โ€“2 nights. Patients are typically swollen and bruised for 14โ€“21 days. Most return to desk work after 3โ€“4 weeks. Final scar maturation 12 months. Strenuous exercise restricted 6 weeks. Numbness in front of the ear common for several months. Full result is judged at 6 months.

Choosing a surgeon

Specialist Plastic Surgeon (FRACS) with specific deep-plane training and high annual volume. Ask about cadaver-course training, fellowship background, and ask to see un-retouched photos of the surgeon's own work at 6+ months post-op. Quality of result varies dramatically with surgeon experience for this technique.

Risk profile for deep plane facelift

How the most-discussed risks for deep plane facelift sit on a frequency-by-severity grid. All cosmetic procedures carry risk; the goal of this view is to help you ask better consent questions, not to dismiss any single complication as unlikely.

Frequency \\ Severity MildModerateSerious
Common
  • Facial nerve injury โ€” temporary or, very rarely, permanent
โ€” โ€”
Uncommon โ€”
  • Permanent scarring
  • Asymmetry between the two sides
  • Hair loss along the incision lines
  • Need for revision surgery
  • Risks of general anaesthesia
โ€”
Rare โ€” โ€”
  • Bleeding and haematoma
  • Infection
  • Skin necrosis (especially in smokers)

Frequency and severity classifications are editorial summaries based on Australian Society of Plastic Surgeons consent literature and TGA / AHPRA guidance. Your surgeon should explain each risk individually as part of the formal informed-consent process โ€” this matrix is a starting point for the conversation, not a substitute.

How to verify any cosmetic surgeon on AHPRA

AHPRA registration is the baseline credential check for any cosmetic procedure in Australia. The five steps below take roughly two minutes and are the single most important due-diligence step before paying a deposit.

  1. 1

    Open the AHPRA register

AHPRA publishes a free public register of every registered medical practitioner in Australia. Open the official register โ€” never rely on a third-party "verification" site.

  2. 2

Search by name and "Medical Practitioner"

Search by the practitioner's full name and select Profession: Medical Practitioner. The register returns active registrations only โ€” if the name does not appear, do not proceed.

  3. 3

Confirm the specialty (or its absence)

Look at the "Specialty" field. A FRACS specialist plastic surgeon will list "Specialist Surgery โ€” Plastic Surgery". Anyone who calls themselves a "cosmetic surgeon" without a recognised specialty is a general medical practitioner under AHPRA โ€” that is a meaningful difference for surgical work.

  4. 4

Check for conditions, undertakings, or restrictions

The register flags any conditions on a practitioner's registration, including AHPRA undertakings or restrictions. Treat any flag as a reason to ask the practitioner directly what it relates to before booking.

  5. 5

    Verify hospital privileges separately

Hospital privileges are issued by the hospital, not AHPRA. For surgical procedures, ask which accredited hospital or licensed day-procedure centre the operation will take place in, and verify the surgeon has admitting rights there.

FAQ

Common questions

How is deep plane different from SMAS facelift?

A standard SMAS facelift dissects above the deep facial fascia and tightens the SMAS layer with sutures. A deep plane facelift goes one level deeper โ€” releasing the retaining ligaments below the SMAS โ€” and repositions the entire midface as a single composite unit. The result tends to look more natural in animation and last longer, but the surgery is more technically demanding.

How long does a deep plane facelift last?

A well-executed deep plane facelift typically rolls back facial ageing by 8โ€“12+ years. The underlying ageing process continues, but most patients continue to look better than they would have without surgery for the rest of their lives.

Why is deep plane more expensive?

Deep plane is technically more demanding, takes longer (often 4โ€“6 hours vs 3โ€“4 for SMAS), and requires more specialised training. Surgeons performing high-volume deep plane facelift typically charge more than those offering primarily SMAS or mini-facelift work.

Is it safer than SMAS facelift?

In experienced hands the safety profile is similar โ€” both are major facial surgeries with established complication rates. The deeper dissection in deep plane is closer to the facial nerve branches but the working plane is more clearly defined. Choose a surgeon experienced specifically with deep plane technique rather than one who occasionally performs it.

Who should perform a deep plane facelift?

Specialist Plastic Surgeon (FRACS) registered with AHPRA, with subspecialty interest in facial aesthetic surgery and specific deep-plane training. Ask about annual deep-plane volume and look at the surgeon's own results photographs.
